List notes on one Sales Coach visit

Notes are activities rows with type note and appointment_id set. Deleted notes (metadata.deleted_at) are omitted. Oldest first. The same handler is mounted at /api/field-sales/rep/appointments/{id}/notes for the iPhone app. The rep path never widens a manager to visits that are not assigned to them. No feature flag.
