Set a per-user feature flag override

Creates or updates a per-user override. An override wins over the workspace flag in either direction — enable a feature for one user while the workspace is off (pilot), or opt one user out of a workspace-wide feature. SaaS platform owners only. CRM_CORE is reserved and rejected with code RESERVED_FEATURE_FLAG.
