Visits happening right now ("In the field"), for managers

One row per rep and visit in the last 12 hours that is checked in (presence) or being recorded (live steps), left-joined. Presence liveness is derived from the last location reading (on_site under 5 minutes, otherwise no_signal with the clock stopped); live steps are live under 2 minutes, paused under 15, else ended. The nudge is derived at read time. When the check-in flag is off, presence is skipped and checkin_enabled is false; recordings still appear. No feature flag of its own; owners, admins and managers only.
